Pregnant/Families who are un-housed with Mental Health Issues or at Risk
The Heartland FSP is a mental health program tailored to address the comprehensive needs of families, including but not limited to those requiring assistance with homelessness or facing the risk of it, individual and family therapy, psychiatric medication assessments and support, treatment for eating disorders, and various other complex mental health services.
If you have clients in mind, please don't hesitate to share their names with me directly for assistance.
To qualify for our services, at least one member of the family must be eligible for Medi-Cal, meet the criteria for medical necessity, and have at least one youth under the age of 21 either residing in the household or planning to return home (the youth doesn't need to be the primary client).
For unlinked clients, kindly complete the attached Checklist and send it directly to Heartland via email at HeartLandFSP@doingwhateverittakes.org, as indicated on the form.
For linked clients, please guide them to the designated program for a transfer of services to the Family FSP, specifying the location (North or South). Additionally, please follow up with an email to me at wakefielda@saccounty.gov so that I can coordinate any necessary transfer of services with the Assigned Contract Monitor.
